Even though this facility's overall grade was not great, it actually earned a good score in our short-term care category. In fact, we awarded it a B for that category, which is one of our better scores. Our short-term care scores are important for patients needing rehabilitation.

This is the percentage of patients who retained mobility levels over time. Preserving mobility is usually a great sign for residents' well-being. While my father in law seemed to get good enough care we will never be sure as there is absolutely no communication at this facility. Not only do they not communicate with the family, they don’t communicate between themselves. Waited to 2 1/2 weeks just to get a meeting to find out what their plan of care was.

This facility is NOT a good fit for ANYONE looking for intermediate level care.
How do you not have a plan to share from the beginning???? Seems like HIPPA laws aren’t being followed when anyone walking by can hear all his medical issues. I would not recommend this place if you are like us and want to know what is going on with your loved ones care.

Despite all of these attempts to communicate his schedule the treating nurse had no knowledge he would be leaving for an appointment. Her response, "well i hope you can still get physical therapy today". Some nursing homes have a mix of Medicare, Medicaid, and private beds. The final item we assessed in this category is the percentage of residents who were able to leave the facility and return to the community.
